ACCEPTANCE OF VOLUNTEER ASSIGNMENT

I, _______________________ agree to volunteer for _____ sessions in the WSYSA/ District III TOPSoccer DAP program. I will adhere to the Code of Conduct and perform all duties assigned to me by the TOPSoccer Coach/Administrator. I understand that failure to do so may result in the TOPSoccer Coach/Administrator not signing my completion certificate.

I understand that failure to complete my sessions will result in further action from the District III Disciplinary Committee.

I understand that poor marks on my Certificate of Completion form in regards to Attitude, Conduct, and/or Participation may result in my reappearance before the Disciplinary Committee and may result in game(s) suspension.
